This role involves working with intellectually and developmentally challenged adults in their homes and community, assisting them with daily routines and recreational activities. There will be challenges in this role, including encountering various types of disabilities and mental health challenges in the individuals served. You will also learn about different agencies and resources to provide direct or indirect support. Responsibilities include working with the individual, their family, and their support network to ensure a safe and healthy home environment, focusing on the individual's needs and preferences. You will also familiarize yourself with community resources, provide training and habilitation to help individuals achieve their desired outcomes, document and complete reporting accurately, and communicate proactively with stakeholders regarding the individuals' progress and any barriers. Additionally, you will participate in required training, team, and company meetings to enhance knowledge, skills, and abilities. You will take ownership of your behaviors and coach others in demonstrating the company's C.A.R.E. values (compassion, adaptability, respect, and empowerment). Ongoing coaching, performance feedback, and recognition for contributions and demonstration of C.A.R.E. values will be provided.
