About The Position

The Coordinated Family Supports (CFS) Program Manager provides leadership, oversight, and supervision of CFS program operations. The CFS Program Manager ensures individualized services are implemented in accordance with client needs, agency standards, and funding source requirements. This position supports client independence, safety, and community integration, while also managing staff, coordinating training, and maintaining regulatory compliance.

Requirements

  • BA Degree in a human services field; OR General studies with a minimum of 5 years of experience in developmental disabilities or related field; OR MSW with experience or knowledge of developmental disabilities; OR An appropriate combination of experience and education may be substituted for some part of the qualifications.
  • Some evidence of competency of training in the areas we teach.
  • Knowledge of Regional centers and systems serving the Developmentally Disabled.
  • Deep understanding of person-centered practices, HCBS Final Rule, and California DDS system.
  • Valid California driver's license.
  • Clean driving record (DMV printout is required).
  • Current vehicle registration and use of a vehicle in good running condition during work hours.
  • Valid automobile insurance for that vehicle and all passengers that meets the minimum coverage requirements of the agency which is (100k/300k bodily injury liability).
  • The willingness and ability to perform the "Essential functions" required within the company, such as office reception, answering phone, new referral consultation, use of computer for projects; typing; duties may be necessary to promote client safety, work.
  • The ability to work collaboratively with consumers, families and co-workers.
  • Ability to work independently and handle multiple priorities.
  • Strong verbal and written communication skills.
  • Tuberculosis Clearance Screen.
  • Current CPR and First Aid certification.
  • Pass criminal background check through fingerprint clearance.
  • Must provide documents required to complete an I-9 application as proof of eligibility to work in the U.S.
  • Understanding of developmental disabilities, mental health, and adult protective services.
  • Familiarity with supervision, training, and healthcare support.
  • Strong organizational, record-keeping, and analytical skills.
  • Proficient in conflict resolution and community navigation.

Responsibilities

  • Identify and coordinate supports necessary for individuals with I/DD to successfully remain in the family home.
  • Conduct intake, assessment, and service planning for eligible individuals and their families.
  • Assist consumers and families in navigating services and supports, including generic and public benefit programs (e.g., Medi-Cal, Medicare, IHSS, Social Security, Section 8).
  • Ensure program is designed and delivered in accordance with person-centered planning principles and tailored to individual and family needs.
  • Monitor compliance with all applicable regulations, including Title 17, Title 22, DDS guidelines, and Regional Centers.
  • Coordinate consistency and quality of services provided by various support professionals.
  • Assist with scheduling and coordination of service delivery, including medical and other essential appointments.
  • Help the client fulfill the short-term goals of the ISP, keeping the long-term goals in mind.
  • Maintain case records according to company procedures.
  • Perform routine home visits, assess needs and coordinate and/or perform assistance with community access to doctor's or health appointments, etc.
  • Re-assess care plan regularly and identify changes in needs for on-going services.
  • Advocate for the client and provide support during Interdisciplinary Team (IDT) meetings and activities in the community.
  • Network in the community to facilitate the development of natural supports for the consumer. initiate and co-facilitate Circle of Support gatherings to assist the consumer in maintaining an effective Circle of Support. Assist the consumer in following up with Circle of Support members to maintain mutually beneficial and long-lasting relationships.
  • Identify and arrange transportation options and backup support services as needed.
  • Support long-term and future planning for consumers, including those residing with aging caregivers.
  • Deliver training and support that enhances the consumer's independence and community inclusion.
  • Ensure that services and supports are culturally and linguistically responsive to the needs of each family.
  • Prepare and submit semi-annual reports documenting service progress and outcomes.
  • Perform all required reporting duties, including Special Incident Reports and Suspected Abuse Reports.
  • Report any suspected abuse within 24 hours to the appropriate agencies and Thrive management.
  • Perform general administrative tasks and other duties as assigned by program leadership.
  • Provide leadership in hiring, scheduling, supervising, and conduct performance evaluations for field-based team members.
  • Perform as a leader to the consumers, staff, providing encouragement, direction and Training.
  • Inform the COO of increased staffing needs and participate in the recruitment, prescreening and interviewing of potential new staff.
  • Enforce and abide by all agency policies and procedures as indicated in the Employee Handbook, Memorandums issued by company managers and Employee Manual.
  • Monitor job safety and supervise CFS's (s) to ensure OSHA requirements are meet.
  • Provide initial and on-going training to CSFs, regarding the proper techniques and procedures for care of the client in accordance with Consumer Orientation checklist.
  • Approve initial training checklists and all updates to checklist.
  • Conduct education and training sessions to team staff members.
  • Provide individual training to staff needing additional instruction and/or counsel.
  • Establish work schedules and assignments for staff, according to workload, space, and equipment availability.
  • Complete ongoing quality assurance.
