About The Position

The Contract Liaison will educate and monitor facilities and staff, providing information on the PACE model/policy and procedure. This individual is responsible for establishing a collaborative working relationship between PACE Southeast Michigan and its external stakeholders, including nursing homes, assisted living facilities, group homes, dialysis centers, and contracted providers. This position is an integral part of the Interdisciplinary Team, providing useful information to improve the admissions process, accurate financial data reporting, and enhance the overall quality of care, service excellence, and relationship building with contracted facilities. The individual is responsible for a proactive approach to educating external stakeholders in the PACE model and coordinating communication with internal staff.

Requirements

  • Must be a Licensed Practical Nurse with current Michigan licensure.
  • Experience working in a Nursing Home required.
  • Good relationship building and customer service skills.
  • Have working knowledge of Utilization Review and Discharge Planning processes.
  • Have working knowledge of Resource Utilization Groups (RUGS).
  • Must be willing to travel between contracted facilities with reliable transportation.
  • Willing to sometimes work flexible hours.
  • Possess excellent organizational and prioritization skills.
  • Creative, detailed-oriented and organized.
  • Excellent written and verbal communication skills with specific ability to maintain accurate records.
  • Strong analytical skills and attention to detail.
  • Working knowledge of computer software for word processing, format development and spreadsheet management.
  • Ability to work with diverse populations/groups/teams.

Nice To Haves

  • MDS experience preferred.

Responsibilities

  • Make a minimum of quarterly visits to nursing homes and other facilities as needed.
  • Establish good working relationships with internal/external staff.
  • Provide education to facility staff relative to PACE, who we are and what we do.
  • Provide care conference information to internal staff to increase communication with external staff at nursing homes.
  • Perform general rounding on all participants to check on their concerns, reporting problems and concerns back to the Interdisciplinary Team (IDT).
  • Serve as a communication resource to the RN Case Managers (RNCMs) and Social Workers.
  • Attend IDT meetings periodically/as needed at each site to retrieve or report the current status of our facility participants.
  • Provide MDS scores to finance in a timely fashion.
  • Ensure RNCMs have access to medical records in facilities.
  • Perform an annual inspection of contracted facilities.
  • Maintain a written record of facility inspections.
  • Provide education and annual reviews with contracted specialists/providers.
  • Escalate facility concerns to the Associate Medical Director or Quality Director.
  • Document all facility concerns and provide in writing to the Quality Director.
  • Other duties as assigned.
