Provides information, advocacy, and service arrangement for consumers and families. This role involves participation in the development of program plans for individuals, monitoring and evaluating these plans, and revising them as necessary. The coordinator will identify and coordinate services for individuals and their families, complete documentation, forms, and reports in accordance with regulations and NLACRC policies. They will serve as an advocate for individuals served by community agencies, schedule and attend interdisciplinary meetings, and identify barriers to service delivery. The position requires implementing culturally sensitive strategies that embody empathy, trust, and respect, and comfort working in underserved and non-traditional office settings. Responsibilities also include rotating officer-of-the-day duties, assisting co-workers, providing case coverage, and offering emergency on-call services when required.
