The Consumer Services Coordinator provides information, advocacy, and service arrangement for consumers and families. This role involves participation in the development of program plans for individuals, monitoring and evaluating these plans, and revising them as necessary. The coordinator will identify and coordinate services for individuals and their families, complete documentation, forms, and reports in accordance with regulations and NLACRC policies and procedures. They will also serve as an advocate for individuals served by community agencies, schedule and attend interdisciplinary meetings, identify barriers to service delivery, and implement culturally sensitive strategies. Additionally, the role includes rotating officer-of-the-day duties, assisting co-workers, providing case coverage, and offering emergency on-call services when required.
