Provides information, advocacy, and service arrangement for consumers and families. This role involves participation in the development of program plans for individuals, monitoring and evaluating these plans, and revising them as necessary. The coordinator will identify and coordinate services for individuals and their families, complete documentation, forms, and reports in accordance with regulations and NLACRC policies and procedures. Additionally, the position serves as an advocate for individuals served by community agencies, schedules and attends interdisciplinary meetings, and identifies barriers to service delivery. The role requires implementing culturally sensitive strategies that embody empathy, trust, and respect, and comfort working in underserved and non-traditional office settings. Responsibilities also include rotating officer-of-the-day duties, assisting co-workers, providing case coverage, and offering emergency on-call services when required.
