The Consumer Services Coordinator provides information, advocacy, and service arrangement for consumers and families. This role involves participation in the development of program plans for individuals, monitoring and evaluating these plans, and revising them as necessary. The coordinator will identify and coordinate services for individuals and their families, complete documentation, forms, and reports in accordance with regulations and NLACRC policies and procedures, and serve as an advocate for individuals served by community agencies. Responsibilities also include scheduling and attending interdisciplinary meetings, identifying barriers to service delivery and implementing culturally sensitive strategies, rotating officer-of-the-day duties, assisting co-workers, providing case coverage as needed, and offering emergency on-call services when required.
