About The Position

The Community Treatment Orders (C.T.O.) Case Manager R.P.N. is an experienced mental health clinician providing registered practical nursing and case management services to clients on Community Treatment Orders in the North East Cluster of the Central East Local Health Integration Network, including the counties of Haliburton, Peterborough, Northumberland, and the City of Kawartha Lakes. The purpose of this role is to promote stabilization and recovery in the community, as well as independent living and prevention of unscheduled return visits to hospital. This includes responsibility for assessment, providing individualized support and intervention to clients within the community, ongoing individual service planning and evaluation, consultation with other service providers and crisis intervention. The Community Treatment Orders (C.T.O.) Case Manager R.P.N. will ensure that the delivery of service, coordination, linkages and advocacy are provided as required. This position will work in close collaboration in a team model with the other R.P.N.s, the regional C.T.O. Coordinator, hospital staff, primary care and any other members of clients’ circles of care.

Requirements

Successful completion of an approved Nursing Degree (R.P.N./R.N.).
Maintains current certificate of competence as an R.P.N. with the College of Nurses of Ontario (CNO) and accepts responsibility for professional competency as a nurse in accordance with CNO’s practice standards.
Maintain membership in good standing with R.P.N.AO and have liability insurance.
Successful completion of the training applicable to this role shall be requirements for continued employment in this position: Health and Safety Training (including but not limited to the following): Workplace Hazardous Materials Information System (W.H.M.I.S.) 2015, Infection, Prevention & Control, Workplace Violence, Accessibility for Ontarians with Disabilities Act, Applied Suicide Intervention Skills Training (A.S.I.S.T.), Verbal Intervention Training, Ontario Common Assessment of Need (O.C.A.N)., Client Record Management System (C.R.M.S.), First Aid/C.P.R., Any training required to maintain good standing with the College of Nurses.
3 or more years of related, preferably clinical, experience as a Registered Nurse/Registered Practical Nurse in a hospital or community setting.
Demonstrated knowledge of the Ontario Mental Health Act, The Health Care Consent Act, social service benefits, related legislation in Ontario and community resources in the Central East Local Health Integration Network.
Demonstrated health assessment skills, sound clinical judgment and decision-making skills.
Demonstrated knowledge of mental illness, concurrent disorders and crisis intervention.
Demonstrated ability, commitment to and knowledge of community health care.
Demonstrated ability to work collaboratively in an interdisciplinary team environment.
Demonstrated interpersonal skills used to work with clients in a diverse practice population.
Demonstrated ability to work independently, as well as, within a team.
Experience with and knowledge of medication injections including administration, documentation, monitoring, and side effects.
Demonstrated knowledge of computer applications (Outlook, M.S. Office, databases).
Valid driver’s license

Nice To Haves

Successful completion of a university degree or college diploma in social/health sciences or related disciplines is an asset.
Additional Nursing certifications, such as Mental Health Nursing.
Experience in Nursing related to the mental health field, including working with addictions, dual diagnosis and/or challenging behaviours.
Experience in Community Nursing.
Intermediate to advanced minus proficiency in French.
Lived expertise with a mental health concern or addiction, either personally or with a family member is an asset

Responsibilities

Provide overall case coordination for clients, in fulfillment of responsibilities outlined on each client’s Community Treatment Orders (C.T.O.).
Complete evidence based Canadian Mental Health Association service plans and crisis plans, in keeping with the C.T.O.
Ongoing assessment, monitoring and supporting clients working towards treatment goals.
Perform nursing duties with clients, including medication administration, long acting injections, monitoring of medication efficacy and side-effects.
Perform mental health status assessments, where applicable and collaborate with psychiatrists within the parameters of the C.T.O.
Assist individuals with system navigation.
Transport/accompany clients to appointments as required.
Provide crisis intervention, advocacy when requested.
Provide referrals to internal and external services.
Work in close collaboration with C.T.O. team members
Provide information about mental health to clients, family members, and other service providers
Complete all required documentation and statistical activities for C.M.H.A. H.K.P.R.
Accurate and timely completion of all other administrative documentation including time sheets, expense claims, vacation and time-off requests, and training requests.
Accurate and timely completion of progress notes, as per agency policy, on client record, in conjunction with data collection processes of service delivery partners, Nursing Assessments, metabolic monitoring, Medication Reconciliation and Best Possible Medication History summaries.
Participate in completion of O.C.A.N. assessments as appropriate.
Develop and promote partnerships with clients, families, service providers, agencies, community partners, hospitals and police.
Participate in community outreach activities through the development of collaborative partnerships and through presentations, education and visits to community agencies and services as appropriate.
On an occasional rotational basis, fill in for other Nurses in other C.M.H.A. H.K.P.R. programs, during periods of absence, to ensure service continuation.
Participate in the agency performance management process/system.
Stay current on relevant legislation and best practices, current research, policies and procedures.
Participate in ongoing professional development and mandatory training.
Actively engage and participate in supervisory process.

Work in compliance with the Occupational Health and Safety Act, applicable regulations and all organizational health and safety requirements and procedures to ensure the health and safety of clients, staff/colleagues, volunteers and students.
Adhere to C.M.H.A. H.K.P.R. client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.
Attends, participates in team/staff meetings, and all-staff meetings as required.
Participates in Agency quality improvement activities, e.g. accreditation projects, committee work, special events, etc.
Participates in quality improvement activities at the team/program level.
Participates in external committees as required.
