About The Position

The My Turn Program, within the Community Support Network in Aging and Disability Services, is currently recruiting for an enthusiastic professional who is dedicated to providing high-quality community navigation services and transitioning youth support to families with children who have intellectual and developmental disabilities (IDD). An ideal candidate will have extensive knowledge about community resources that are available to support children with IDD, the transitioning youth process, experience conducting outreach, and have strong communication skills. The Program Specialist II will serve as a Community/Transitioning Youth Navigator in the My Turn Program. This position will provide navigation/case management services and coordinate resources to children/youth with developmental and intellectual disabilities (ages 3 -21). Duties to include, but are not limited to: Assist children and youth with intellectual/developmental disabilities and their families by connecting them to State and local resources, programs and/or services in the community Respond to requests for assistance and provide enhanced assistance, guidance and navigation support as needed, including transitioning youth Serve as the lead on Transitioning Youth issues for the program, including establishing and maintaining strong partnerships with a variety of community stakeholders Provide more intensive case management assistance and navigation support as needed Travel in the community to meet with community partners, families and to monitor services. Track client funding allocations, process invoices, and follow up on client assistance payments to meet fiscal year demands. Strong ability to communicate effectively and professionally in a variety of formats, including strong presentation skills. Work Schedule: Flexibility is required as this position requires conducting presentations to the community and participating in community outreach events during evenings and weekends. Telework: This position may be eligible for limited hybrid telework after the initial training period and in accordance with departmental policies and operational needs. Driver’s License: This position requires possession of a valid driver's license to travel throughout Montgomery County to meet with families, visit community partners, attend outreach events, monitor client services, and perform other work at locations outside the primary office. Bilingual applicants in the Spanish/English languages are encouraged to apply. If the candidate selected for this position possesses multilingual skills, they will be required to successfully pass an examination assessing oral communication, reading comprehension, and interpreting skills. For Advanced proficiency, the candidate must also pass an examination requiring translating text from Spanish to English and vice versa. Background Investigation: A criminal background check will be conducted on the selected candidate prior to appointment and will be a significant factor in the hiring decision.

Requirements

Two (2) years of professional experience in case management specifically working with individuals with developmental disabilities
Graduation from an accredited college or university with a Bachelor's Degree.
Possession and maintenance at all times of a valid Class "C" (or equivalent) driver’s license from the applicant's state of residence.
Completion of a pre-employment Medical History Form to assess your ability to safely perform the essential duties of the role.

Nice To Haves

Experience providing case management or navigation support to children or youth with intellectual or developmental disabilities.
Experience supporting families and youth through the Transitioning Youth process, including resource coordination and service planning.
Experience developing or managing tracking systems for client eligibility, service plans, or fiscal monitoring.
Experience assisting clients or families during crisis situations and applying effective problem-solving strategies.
Experience communicating and collaborating professionally with families, nonprofit organizations, school partners, and community groups.
Bilingual ability in Spanish.
