The Community Supports Navigator provides support, assistance and instruction to individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian. Examples of support include but are not limited to: assistance with apartment maintenance, money management, medical appointments, advocacy, meal preparation and nutrition, socialization, crisis intervention and skill acquisition. The Case Worker is also responsible for providing quality documentation, support of corporate rep-payee responsibilities and may be asked to provide surrogacy supports. This role is responsible for creation and implementation of ISP, Behavior Plans, Medication Treatment Plans, Assessments, and any other applicable documentation related to successfully supporting individuals in the community within designated time frames. It also involves providing training, modeling and assistance in activities of daily living and health maintenance in order to enhance skill acquisition and maintain independent living, including but not limited to; meal preparation and nutrition, apartment maintenance, exercise, socialization and utilizing public transportation/ADA. The Navigator provides case management for individuals including support, assistance and instruction to develop independence and achieve goals set in ISP. They also work collaboratively with Ascentria Leadership and Finance to ensure compliance with all rep payee responsibilities such as but not limited to: Financial Training Plan, Funds Management Plan, annual reporting, and bill payment.
