The Community Supports Navigator provides support, assistance and instruction to individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian. Examples of support include but are not limited to: assistance with apartment maintenance, money management, medical appointments, advocacy, meal preparation and nutrition, socialization, crisis intervention and skill acquisition. The Case Worker is also responsible for providing quality documentation, support of corporate rep-payee responsibilities and may be asked to provide surrogacy supports. This role involves creating and implementing various plans (ISP, Behavior Plans, Medication Treatment Plans, Assessments), providing training and assistance in daily living activities and health maintenance, case management, ensuring compliance with rep payee responsibilities, providing transportation, assisting with public benefits, helping with moves, offering problem-solving information, modeling social behavior, and serving as an advocate.
