About The Position

The Case Worker provides support, assistance and instruction to individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian. Examples of support include but are not limited to: assistance with apartment maintenance, money management, medical appointments, advocacy, meal preparation and nutrition, socialization, crisis intervention and skill acquisition. The Case Worker is also responsible for providing quality documentation, support of corporate rep-payee responsibilities and may be asked to provide surrogacy supports.

Requirements

  • Individuals with a developmental disability (ID/DD) and/or autism spectrum disorder (ASD) living independently or at home with a legal guardian.
  • Quality documentation.

  • Creation and implementation of ISP, Behavior Plans, Medication Treatment Plans, Assessments, and any other applicable documentation.
  • Training, modeling and assistance in activities of daily living and health maintenance.

  • Meal preparation and nutrition.
  • Apartment maintenance.
  • Exercise.
  • Socialization.
  • Utilizing public transportation/ADA.
  • Case management for individuals.
  • Support, assistance and instruction to develop independence and achieve goals set in ISP.
  • Compliance with all rep payee responsibilities such as but not limited to: Financial Training Plan, Funds Management Plan, annual reporting, and bill payment.
  • Transportation for and escort individual to and from appointments as needed.
  • Assist in obtaining public benefits such as but not limited to: Social Security, Mass Health, ADA transportation, food stamps and fuel assistance.
  • Assist individuals when moving from one location to another.
  • Moving furniture and/or boxes.
  • Using personal vehicle to transport client personal belongings.
  • Basic problem solving information.
  • Appropriate social behavior.
  • Advocacy for individuals at various community levels.
  • Evaluate effectiveness of advocacy efforts and make appropriate adjustments.
  • Regular updates and communication with Program Manager, Clinical Coordinator, and DDS staff.
  • Prepare required paperwork and submit by deadline.
  • Attend and participate in staff, team and PQI process meetings.
  • Respect the rights, privacy and the dignity of the client.
  • Adhere to all contractual regulations.
  • Professionalism and appropriate business attire.

Responsibilities

  • Responsible for creation and implementation of ISP, Behavior Plans, Medication Treatment Plans, Assessments, and any other applicable documentation related to successfully supporting individuals in the community within designated time frames
  • Provide training, modeling and assistance in activities of daily living and health maintenance in order to enhance skill acquisition and maintain independent living. These may include but are not limited to; meal preparation and nutrition, apartment maintenance, exercise, socialization and utilizing public transportation/ADA.
  • Provide case management for individuals including support, assistance and instruction to develop independence and achieve goals set in ISP.
  • Provide training, modeling and assistance in activities of daily living and health maintenance in order to enhance skill acquisition and maintain independent living. These may include but are not limited to; meal preparation and nutrition, apartment maintenance, exercise, socialization and utilizing public transportation/ADA.
  • Work collaboratively with Ascentria Leadership and Finance to ensure compliance with all rep payee responsibilities such as but not limited to: Financial Training Plan, Funds Management Plan, annual reporting, and bill payment
  • Provide transportation for and escort individual to and from appointments as needed.
  • Assist in obtaining public benefits such as but not limited to: Social Security, Mass Health, ADA transportation, food stamps and fuel assistance.
