About The Position

The Community Support Worker mentors and advocates for adults and/or youth, providing one-on-one support in the field to develop life skills, manage mental health, and build independence. The role involves helping participants work toward personal recovery goals and access community resources, with work across Wicomico, Worcester, and Somerset counties. The position is full-time, community-based, and requires travel; mileage is reimbursed.

Requirements

High school diploma and one year of work experience in a supervised mental health setting, or an associate or bachelor's degree in a mental health- or human-services-related field.
Must possess or obtain First Aid and CPR certification.
Must be willing to submit to and pass a CJIS and Child Protective background check.
Active, unrestricted driver's license and reliable personal vehicle.

Opportunities may be available to provide supervision and training hours toward CPRS/LCSW certifications.

Responsibilities

Provide one-on-one coaching in financial management, job readiness, health education, social skills, and independent living.
Collaborate with clients to develop and implement Individual Rehabilitation Plans (IRPs) focused on personal recovery goals.
Accompany and support clients as they access community resources, including medical appointments, social services, and vocational training.
Maintain high-quality clinical progress notes for each session that meet company requirements and Medicaid compliance standards.
Identify potential barriers to recovery and work with the team to intervene before a situation becomes a crisis.
