About The Position

This full-time, field-based position supports adults and/or youth in Wicomico, Worcester, and Somerset counties. The Community Support Worker works one-on-one with participants as a mentor and advocate, helping them develop life skills to manage their mental health, build independence, and pursue long-term recovery. The role includes coaching in practical skills, helping participants access community resources, and documenting services in accordance with company and Medicaid requirements. Travel is required, with mileage reimbursement. Work is in person and community based, with some in-office days.

Requirements

High school diploma and one year of work experience in a supervised mental health setting, or an associate or bachelor's degree in a mental health- or human services-related field.
Must possess or obtain First Aid and CPR certification.
Willingness to submit to and pass CJIS and Child Protective background checks.
Active, unrestricted driver's license and reliable personal vehicle.

Nice To Haves

Opportunities may be available to provide supervision and training hours toward CPRS or LCSW certifications.

Responsibilities

Provide one-on-one coaching in financial management, job readiness, health education, social skills, and independent living.
Collaborate with clients to develop and implement Individual Rehabilitation Plans focused on their recovery goals.
Accompany and support clients as they access community resources, including medical appointments, social services, and vocational training.
Maintain high-quality clinical progress notes for every session that meet company requirements and Medicaid compliance standards.
Identify potential barriers to recovery and work with the team to intervene before situations become crises.
