About The Position

This is a temporary position for a Community Support Worker II in Calgary. The role involves providing quality care and professional direct client support in alignment with the Excel Society's values and the "Client's Rights" statement. Responsibilities include assessing client needs, accompanying clients to assessments, collecting data on behaviors and skills, ensuring health and medical needs are met, assisting with financial support, implementing daily living skills, and being a supportive team member. The role also involves contributing to the development of support plans, facilitating information exchange during shift changes, and liaising with families, guardians, and other professionals.

Requirements

  • A certificate in disability studies and/or social work from a recognized institution. Equivalencies may be considered.
  • Knowledgeable and experienced with various diagnosis surrounding persons with disabilities.
  • Previous experience in disability services or human services is required
  • First Aid CPR Level C Certificate is required
  • Police Check with Vulnerable sector is mandate
  • A caring, reliable, and person-centered approach
  • Strong communication skills are required
  • Possess well developed computer use skills.

Responsibilities

  • Providing quality care and professional direct client support in a manner which reflects the values of the Excel Society and honours the “Client’s Rights” statement.
  • Contributing to assessing client(s)’ service needs and/or accompanying client(s)’ to available relevant assessment services.
  • Utilizing appropriate data collection methods to collect information regarding behaviours of concern or to teach new skills.
  • Ensuring that health and medical needs of the clients are met.
  • Assisting clients with their financial support needs.
  • Implementing all aspects of daily living skill ensuring client(s)’ needs are met as designated.
  • Being a supporting, positive, and professional team member.
  • Contributing as a part of the team to the development of an Individual Support Plan, Essential Lifestyle Plan, or Person Centered Plan (as appropriate) for each client they directly support.
  • Ensuring that all pertinent information is exchanged between staff members during shift exchanges.
  • Liaising with families, guardians, professionals and other resource members of the client(s)’ support network, communicating relevant client information to other team members.
