About The Position

Metropolitan Family Services (MFS) is seeking a Community Support Team (CST) Team Lead for their Merrionette Park Office. Requirements

  • Bachelor’s degree in a human services field or a bachelor’s degree in any field with two years of documented clinical experience in a mental health setting, required.
  • Must obtain and maintain IM+CANS certification, required.
  • Requires mobility and willingness to work with a community-based caseload.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Must work some evenings and weekends and possible on-call.
  • Satisfactorily pass all relevant screening and criminal background checks.
  • Driving for work required with transportation of clients preferred in a personally owned vehicle.

Nice To Haves

  • Prior experience with Rule 132/140 or CST/ACT (Assertive Community Treatment) models strongly preferred.
  • Master's Degree in Social Work or Counseling from an accredited college or university. Must have applied to take LSW or LPC examination, preferred.
  • Certified Alcohol and Drug Counselor (CADC) or experience working with clients experiencing substance use preferred.
  • IM+CANS certification, preferred.

Responsibilities

  • Assist in conducting IM+CANS assessments and implementing treatment plans.
  • Perform open access as assigned.
  • Provide case management, support financial budgeting, assist with public benefits (Social Security, Medicaid), therapy/counseling, advocacy, health education, and community support services.
  • Provide individual and group counseling, medication training and monitoring, skills training, and crisis intervention.
  • Respond to client and family needs outside of scheduled sessions, including during crises.
  • Identify and respond to changes in client needs, including decompensation or risk of hospitalization, and adjust levels of care accordingly in consultation with supervisor.
  • Work with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
  • Coordinate care with family members, internal providers (medical, dental, SA/DV), and external partners such as schools, DCFS, hospitals, and community organizations.
  • Learn and utilize agency and community resources.
  • Assess community conditions affecting clients to identify resource gaps and problems.
  • May serve on regional or agency-wide committees.
  • Meet agency expectation for quantity of direct service and maintain quality and fidelity to evidence-based practices.
  • Maintain timely and accurate client records and any necessary agency and CCBHC required reporting.
  • Participate in agency and site-based required meetings and trainings.
  • Work effectively with diverse staff and service population.
  • Other duties as assigned.
