About The Position

Carolina Outreach is seeking a full-time Peer Support Professional to join our Community Support Team (CST). In this role, you will provide clinical services to all clients receiving CST team services, including but not limited to: tenancy support interventions using the Permanent Supportive Housing model; service coordination, including working with the CST team leader to monitor each individual’s clinical status and response to treatment; symptom assessment and management; individual supportive counseling; providing activities-of-daily-living, social, interpersonal relationship, and leisure-time-activity services; support services or direct assistance to ensure that clients obtain the basic necessities of daily life; and education, support, and consultation to individuals’ families and other major supports. Carolina Outreach has been assisting children, families, and adults across North Carolina with their behavioral health needs since 2003. We are a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need utilizing an equitable and inclusive approach. We focus on helping our clients tap into and develop their inherent strengths with the goal of supporting the highest quality of life possible for all we serve.

Requirements

  • Must have completed certification program for Peer Support Specialists and have at least two years of sustained recovery from any identified mental health or substance use issues.
  • Must become NVCI certified and must have received training in the population served and client rights/confidentiality.
  • A valid driver’s license and current automobile insurance is also required.
  • Must be authorized to work in the United States.

Responsibilities

  • Assist in identifying, planning, scheduling, and implementing services for the client.
  • Assist in the development of transition plans when clients move in and out of services.
  • Act as an advocate in the community.
  • Provide any written documentation as required. This will include treatment notes, goals, and plans for the medical record (e.g., service notes, input into person-centered plan, etc.)
  • Be available to respond as needed to clients and co-workers 24/7/365.
  • Must be free from all illicit substance use.
  • Demonstrate adherence to the treatment models within CST such as Permanent Supportive Housing, CBT strategies, MI, and Harm Reduction.
  • Provide individual, family, and/or group therapy sessions
  • Maintain accurate and timely clinical documentation
  • Collaborate with care teams as needed
  • Maintain compliance with all clinical and regulatory standards
  • Participate in supervision (if applicable based on licensure level)
