Community Support Specialist

About The Position
This is a professional-level position providing social services to people with serious mental illness. The specialist provides specific, measurable, individualized services focused on helping each person remain in the community and identify and access needed services. The role includes monitoring consumer progress, assessing social, medical, and psychiatric needs, connecting consumers with appropriate services, and maintaining required documentation and case records.
Requirements
- Bachelor’s degree in a mental health-related field.
- Must receive Mississippi Department of Mental Health certification as a Community Support Specialist.
- Must pass a criminal background check.
- Must have a valid Mississippi driver’s license.
Responsibilities
- Maintain documentation showing that duties are completed in accordance with Mississippi Department of Mental Health Operational Standards.
- Maintain required training, continuing education, qualifications, and certifications.
- Monitor consumer progress through face-to-face contact according to each consumer’s level of functioning.
- Collect data and prepare consumer histories based on interviews with consumers, family members, and collateral contacts.
- Provide follow-up monitoring of psychotropic medications.
- Determine consumers’ social, medical, and/or psychiatric needs.
- Locate and refer consumers to appropriate agencies or facilities for services.
- Cooperate with representatives of agencies receiving referrals.
- Maintain contact with other service agencies.
- Maintain case records for assigned work.
- Serve as a public relations representative for community support services.
- Conduct home visits as necessary to monitor consumers.
- Provide at least six hours per day of face-to-face direct service to consumers.
- Provide transportation for consumers as needed.
- Perform other duties as assigned by supervisors.
