About The Position

Community Support Services offers a variety of supports to help Burlington County adults with a serious mental illness remain living successfully in the community. Services include case management, housing assistance, supportive counseling, advocacy, service linkages, illness management, peer support, life skills, education/employment assistance, transportation and 24-hour on-call support. Enhanced Community Support Services is also available for adults being released from state psychiatric hospitals. This program offers a higher level of support and case management.

Requirements

• Bachelor’s Degree from an accredited college or university in a helping profession, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.
• If Bachelor’s Degree is in a field other than a helping profession, must have a minimum of two years of professional experience in a community behavioral healthcare setting.
• Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience.

Responsibilities

• Contributing to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans
• Therapeutic rehabilitative skill development
• Implementation of Illness Management and Recovery (IMR) principles and interventions
• Coordinating and managing services, both mental health and physical health
• Crisis intervention, as needed
• Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration
• Maintaining own appointment schedule and meeting work productivity requirements
• Other duties as assigned by management staff
• Productivity requirement of one hour face to face contact per shift
