Your Role at Sana Lake Sana Lake is expanding rapidly and is seeking a Community Support Specialist who is passionate about helping individuals and families recover!
Requirements
- Have a Bachelor's Degree or Associate's Degree
- Have or will obtain a valid State of Missouri Class E Driver's License
Nice To Haves
- Have a minimum of 1 year of case management with substance use treatment experience, preferred
Responsibilities
- Find and link members with internal and external resources to meet their needs.
- Assist members in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the member's progress in recovery.
- Collaborate with the Treatment Team to formulate the Master Treatment Plan and assist in coordinating discharge planning.
- Maintain communication between treatment team members, family, referral sources, and other service providers as needed.
