This is a community-based position within the Assertive Community Treatment Team (ACTT) which serves adults 18 years of age and older. ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and are determined to require a high level of service needs. The Community Support Specialist II partners with individuals to accomplish their recovery goals, conducts various assessments, and collaborates to create individualized, person-centered treatment plans. They utilize motivational interventions, provide psycho-educational rehabilitation services, and support individuals in linking with community supports. The role also involves developing crisis and safety plans, providing skills teaching for daily living, participating in family education, and documenting client progress. Attending staff meetings, trainings, and other assigned meetings is also a key responsibility.
