Bachelor-level Targeted Care specialist candidate needed to fill a full-time, clinical position on a dynamic, interdisciplinary Assertive Community Treatment (ACT) team. Targeted Care Specialists assist adult clients with severe and persistent mental illness in their home and in the community apply skills learned in therapy re: coping skills to address symptoms, addressing activities of daily living, skill-building around independence, medication adherence, and supporting them to local resources. Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy. Fundamental computer experience to process e-mail, electronic medical record, and Microsoft Teams required. Willing to train the right applicant.

Requirements

  • Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy.
  • Fundamental computer experience to process e-mail, electronic medical record, and Microsoft Teams required.
  • Experience working with people with severe mental illness preferred.
  • Ability to work as an effective team player is essential.
  • Able to work independently and build own work schedule week to week.
  • Position requires a bachelor’s degree in psychology, social work, or related field from an accredited college or university.

Responsibilities

  • Work with clients to accomplish tasks (such as grocery shopping, applying for benefits, securing housing, following through with medical treatment, etc.) through the successful use of skills (such as interpersonal, behavioral activation, distress tolerance, problem solving, and self-efficacy).
  • Work closely with Masters level clinical case managers, nurses, and psychiatrists on a multidisciplinary team, display critical thinking, and strong organizational abilities.
  • Complete progress notes on all clients seen, attend treatment team meetings, supervisions, and group supervisions.
