The Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older. ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and are determined as requiring a high level of service needs. The Community Support Specialist II will partner with individuals to accomplish their recovery goals, conduct co-occurring screens, C-SSRS, and functional assessments. This role involves collaborating with individuals to create individualized, person-centered treatment plans, documenting client progress, and using motivational interventions to assist clients in meeting their goals. The specialist will provide psycho-educational rehabilitation services, support individuals in linking with natural or community supports, and develop crisis and safety plans. Additionally, the role includes providing skills teaching for areas such as housing, utilities, transportation, nutrition, social engagement, problem-solving, and family care, as well as participating in family education and support activities. Documentation of client progress, attendance at staff meetings, trainings, and other assigned meetings are also key responsibilities. The position may involve other assigned duties.
