About The Position

Community Support Specialist (Housing Focus) in Indianapolis, IN, within the Community-Based Services department. This full-time role is ideal for recent graduates in Social Work, Psychology, Criminal Justice, or Human Services seeking a career in behavioral health. The specialist will work directly with individuals facing mental health and/or substance use challenges, many of whom are homeless or at risk of homelessness. The primary goal is to connect clients with housing, healthcare, and essential community resources to foster independence and long-term success. This is a hands-on, community-based position requiring strong relationship-building and advocacy skills.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services or related field OR Relevant experience working with individuals with mental health conditions, substance use disorders, or homeless or at-risk populations
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to travel throughout the community to meet with clients
  • Must meet background screening requirements
  • Ability to complete required onboarding and ongoing training

Responsibilities

  • Help clients find, secure, and maintain housing
  • Build and maintain relationships with landlords and housing partners
  • Assist with housing applications, resources, and financial assistance options
  • Provide community-based case management to individuals in supportive housing
  • Connect clients to healthcare, employment, education, and social resources
  • Advocate on behalf of clients to ensure access to needed services
  • Help clients build independent living skills (budgeting, hygiene, daily routines)
  • Support development of coping skills and recovery strategies
  • Provide education on mental health, substance use, and wellness
  • Work closely with treatment teams, healthcare providers, and community partners
  • Participate in treatment planning and team meetings
  • Coordinate services to support long-term success and stability
  • Maintain accurate and timely documentation of services
  • Follow up with clients and referral sources to support engagement
  • Assist with discharge and long-term recovery planning
  • Support clients during crisis situations
  • Help connect individuals to appropriate crisis resources and services
