About The Position

The Community Support Facilitator (CSF) oversees a caseload of clients in our Supported Living Services (SLS) program, and is responsible for managing the daily support of individuals with developmental disabilities living in their own homes. The Community Support Facilitator (CSF) will advocate for and support our clients to lead meaningful lives of their choosing, make informed decisions, and participate in their communities. The CSF will train and supervise Supported Living staff in their duties, and complete administrative tasks for our clients and employees. The Community Support Facilitator (CSF) will advocate on behalf of our clients and assist them to navigate government agencies to access public benefits. Advocating for people with developmental disabilities, supporting them to be as independent as possible, and breaking barriers to ensure community inclusion require a collaborative approach. Therefore the CSF will have a strong support system within the agency, including a dynamic team of fellow Facilitators and ongoing training. Since East Bay Innovations is a human services and social services nonprofit with a commitment to social justice, the facilitator will work with people who believe in advocacy and who also share those passions.

Requirements

  • 5 years experience working with individuals with disabilities
  • Two or more years in human services, social services, or social work
  • Supervisory experience
  • Bachelor’s degree or equivalent work experience
  • Knowledge in Google Suite and Microsoft Office programs
  • A commitment to social justice and human rights
  • Ability to think strategically, creatively problem solve, exercise good judgment and lead change
  • Excellent analytical and abstract reasoning skills
  • Proven ability to work in a dynamic team and build strong relationships
  • Able to lead a diverse workforce team around a common goal
  • Must pass a Live-scan DOJ background check and DMV pull
  • California driver's license
  • Proof of auto insurance
  • Access to a vehicle

Responsibilities

  • Oversee a caseload of clients in the Supported Living Services (SLS) program
  • Manage the daily support of individuals with developmental disabilities living in their own homes
  • Advocate for and support clients to lead meaningful lives of their choosing, make informed decisions, and participate in their communities
  • Train and supervise Supported Living staff in their duties
  • Complete administrative tasks for clients and employees
  • Advocate on behalf of clients and assist them to navigate government agencies to access public benefits
  • Provide emotional support and companionship
  • Get exercise and walk around clients' neighborhoods
  • Connect clients with friends and loved ones
  • Assist with meal planning and cooking meals
  • Perform light cleaning around clients' homes
  • Provide personal/intimate care
  • Assist with wheelchair transfers
  • Assist with grocery shopping and other errands
  • Support clients to attend medical appointments and coordinate care
  • Hold the 24 hour Emergency Cellphone several times per month
