This position involves teaching clients essential life skills to promote independence in the community. Responsibilities include training in areas such as accessing community resources, medication adherence, interpersonal skills, behavioral support, crisis coping, emotional regulation, transportation access, financial management, and independent community shopping. The role also requires completing documentation in the EMR within 24 hours of service delivery, conducting activities to support maximum independence, enhancing community living through skills training, cueing, or supervision as per treatment plans, and maintaining all required trainings and supervision. Participation in therapeutic rehabilitation and 1915i Rise services is also part of the role.
