About The Position

Kentucky River Community Care is seeking a Regular Full-Time Community Support Associate/Life Skills Coach/Transporter for their Breathitt County Health & Wellness location in Jackson, KY. This role involves teaching clients essential life skills to promote independence within the community, including accessing resources, medication adherence, interpersonal skills, behavioral support, crisis coping, emotional regulation, transportation access, financial management, and independent shopping. The position also requires timely documentation in the EMR, participation in therapeutic rehabilitation and 1915i Rise services, and maintaining all required trainings and supervision.

Requirements

• High School diploma or GED
• One year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder.
• Valid driver's license with 2 years of driving experience.
• Good computer skills.
• Ability to work independently.
• Good organizational and time management skills.

Responsibilities

• Teaching clients skills such as utilizing/accessing community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
• Complete documentation in EMR within 24 hours of service delivery.
• Conduct activities necessary to allow individuals to live with maximum independence in the community.
• Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.
• Maintain all trainings and required supervision.
• Participate in therapeutic rehabilitation and 1915i Rise services as needed.
