About The Position

The Community Specialist assists with implementing each person's ISP/Day Habilitation Plan by planning, providing, and documenting community integration experiences. The role uses community-based resources and supports as appropriate to the plan and provides direct care, training, and supervision as needed. The position serves the Oneida County—Utica and Clinton area.

Requirements

• An associate degree in human services or a related field is preferred; alternatively, a high school diploma or GED with specialized training, or a year of experience working with people with developmental disabilities, in an educational setting, or in a health care facility.
• Ability to transfer and position individuals and manipulate equipment such as wheelchairs.
• Valid New York State driver's license.

Responsibilities

• Ensure assigned individuals are supervised at all times.
• Provide transportation for community outings in accordance with established policies and procedures for agency vehicles.
• Promote and follow person-driven choices in independence, individualization, inclusion, and productivity, as well as the Five Governing Principles.
• Provide the assistance required with activities of daily living, including toileting, eating, ambulation, and lifting.
• Assist in implementing ISP/Day Habilitation Plans by planning, providing, and documenting community integration experiences using appropriate community-based resources and supports.
• Provide direct care, training, and supervision to the extent required.
