About The Position

The Community Services Coordinator may carry a caseload of under 20 clients and supports the day to day operations of the care coordination team, tracking utilization of assigned staff, implementing program strategies, and ensuring continuous improvement related to the delivery of services. The Community Services Coordinator reports to the Housing & Care Manager to develop and implement compliance in program protocols, resolve complex issues, and ensure reporting data is accurate and compatible with third party stakeholders. The Community Services Coordinator works closely with AOD Counselors & ARS Care Coordinators in support of patient needs at all stages and levels of care.

Requirements

  • AA in Addiction Medicine or 3 years experience in a related field.
  • High school diploma or GED
  • Proficiency in electronic health record systems or other platforms required for ECM documentation.
  • Completion of required CalAIM training within 90 days of hire.
  • Must have the ability to obtain and maintain CDCR clearance.
  • Knowledge of offender populations, criminal subcultures, and cultural differences, substance abuse assessment tools, therapeutic community concepts, familiarity with operations of Day Reporting Centers, Parolee Service Centers, Residential Multi-Service Centers, Specialized Treatment for Optimized Programing (STOP), Female Offender Treatment and Employment Programs, outpatient and residential treatment programs, and any additional transitional services developed by CDCR or the Counties.
  • A valid California driver’s license
  • Able to lift 51 lbs
  • CPR & First Aid Certification
  • Negative or noncommunicable TB test

  • Bachelor’s Degree and/or CRMA/CCAPP certification preferred and one (2) years of related work experience.

Responsibilities

  • Perform Enhanced Care Management, Community Supports, & Housing Navigation duties
  • Prepare for and participate in individual and group supervision, case review, and staff meetings
  • Help clients in their recovery from substance use and co-occurring disorders through role modeling and trauma-informed support using the values, principles, and tools of the treatment program.
  • Act as Lead Client Care Coordinator providing additional knowledge, expertise, and guidance to Level I & II Client Care Coordinators.
  • Implement and manage grant awards in support of program priorities.
  • Document and update new program protocols as required.
  • Oversee compliance with CalAIM requirements for ECM programs, including documentation audits and reporting outcomes.
  • Network with city, county, state, and other third-party stakeholders in support of program compliance and development goals.
  • Collaborate with Managed Care Plans (MCPs), community-based organizations (CBOs), and other stakeholders to ensure effective implementation of CS services.
  • Respond to client-related emergencies.
  • Develop program census to meet company treatment goals.
  • Train new hires as required.
  • Participate in ongoing training and professional development.
  • Maintain an active weekly schedule of meetings and advocacy for all patients assigned to caseload.
