About The Position

Blue Sky is seeking an experienced professional to serve as a Community Services Coordinator and Designated Coordinator within its Minnesota 245D program. This role involves coordinating community-based services, supporting and coaching employees, collaborating with families and support teams, and ensuring individualized services are delivered safely, consistently, and according to plan. The position requires a hands-on approach to leadership, organization, problem-solving, and effective communication, with a focus on person-centered care, proper documentation, and regulatory compliance.

Requirements

  • Ability to successfully complete a Minnesota DHS background study
  • Valid driver's license and driving record that meets Blue Sky requirements
  • Reliable, insured transportation
  • Strong written and verbal communication skills
  • Comfort using computers, email, Microsoft Office, and electronic documentation systems
  • Ability to organize multiple priorities, meet deadlines, and follow through on responsibilities
  • Ability to lead employees while working collaboratively with individuals, families, guardians, coworkers, and other professionals
  • Bachelor's degree in a human services-related field and at least 1 year of qualifying direct-care experience
  • Associate degree in a human services-related field and at least 2 years of qualifying direct-care experience
  • Diploma in a human services-related field and at least 3 years of qualifying direct-care experience
  • At least 50 hours of education and training related to human services and disabilities and at least 4 years of qualifying full-time direct-care experience under appropriate supervision

Responsibilities

  • Supporting and coaching employees who provide direct services
  • Helping employees understand expectations and solve problems
  • Coordinating services based on each individual's needs, goals, and support plan
  • Communicating with individuals, families, guardians, team members, and other professionals
  • Participating in service planning and support team meetings
  • Developing, implementing, and monitoring individualized support strategies
  • Training employees on participant-specific needs and service expectations
  • Reviewing documentation and following up when something is incomplete or needs attention
  • Responding to changes in staffing, services, participant needs, or other operational priorities
  • Identifying concerns and determining what needs to happen next
  • Helping ensure services meet Blue Sky standards and Minnesota 245D requirements
  • Working with other Blue Sky leaders to improve service quality and participant outcomes
