To educate persons diagnosed with a dementia and caregivers, facilitate access to available public and private resources and to provide support services including counseling and care planning. This role involves understanding mental health issues common among persons with dementia and caregivers, utilizing effective non-pharmacological interventions for managing dementia-related behaviors and common mental health disorders, and implementing interventions to reduce caregiver burden and improve their physical and mental health. The coordinator will also identify and utilize available educational and informational resources, interact with patients and families in a manner attuned to their individual needs, health literacy levels, and home environments, and assess any related financial burdens that may impact service accessibility or affordability. Additionally, the role requires efficient administration and interpretation of standardized assessment tools, integration of assessment results into care plans, and assessment of caregiver burden, mental health, knowledge, and capacity. The coordinator will provide for the social needs of patients/families, complete psychosocial assessments, participate with team members in care planning, and find alternatives to home care when indicated. Documentation, compassionate treatment, and adherence to the care plan are essential, as is monitoring and documenting patient response. Establishing rapport, adapting communication styles, listening, and responding to patient needs are key. The role also involves recognizing age-specific responses to treatment, providing effective interventions, respecting patient confidentiality and privacy, supporting social, spiritual, and cultural needs, and managing difficult or emotional patient situations.
