About The Position

The Community Resource Coordinator serves as a vital link between OBCC families and the essential resources they need to thrive. This role is responsible for transforming positive SDOH screens into meaningful support by conducting patient-centered follow-up assessments to clarify family needs and prioritize resource allocation. The Coordinator provides proactive case management for families with complex needs, ensuring continuity of care while paving a clear path toward long-term independence and graduation from our services. The Coordinator facilitates family access to and navigation of concrete needs requests, health insurance (Medicaid), and social welfare programs (SSI, DDA, SNAP, TANF). By staffing the clinic "Marketplace" window and managing clinical referrals, this role ensures families have a reliable partner to help them overcome systemic barriers and access housing, transportation, and food assistance.

Requirements

  • High School Diploma or another combination of education and experience that would provide similar competency may be considered
  • Minimum 2 years of experience in social services, community health, or youth advocacy, with prior experience navigating complex systems and community resources
  • Experience providing consultation to employees/staff at different levels
  • Access to transportation to attend occasional off-site visits, trainings, or meetings

Nice To Haves

  • Being bilingual Spanish, Cantonese, Vietnamese and/or Somali is a highly preferred skill for the position
  • Prior experience in case management serving complex families.
  • Knowledge of local resources for families with young children, preferably resources in King County
  • Interest in continuing to learn and explore how health equity, diversity, and inclusion impacts the work at Seattle Children’s
