About The Position

The Lead Community Resource Coordinator (CRC) acts as a subject matter expert and primary support for the CRC team, ensuring families at OBCC are seamlessly connected to essential resources. In addition to performing core resource coordination—such as conducting patient-centered SDOH follow-up assessments, managing the "Marketplace" window, and assisting with Medicaid and social welfare enrollment (SSI, DDA, SNAP, TANF)—the Lead is responsible for onboarding, orienting, and mentoring staff. The Lead ensures operational efficiency by addressing day-to-day workflow issues, overseeing the tracking of concrete needs, and assisting in the development of new standard work and processes to improve the experience of care for underserved families.

Requirements

  • Bachelor’s Degree in Social Science or a related field; or a combination of education and experience providing similar competency.
  • Minimum 3+ years of experience, including leading case management for complex families and coordinating across multiple systems.
  • Demonstrated experience providing consultation or mentorship to employees/staff at different levels.
  • Proven ability to work successfully in diverse communities with an understanding of systems of oppression and a commitment to anti-racism.
  • Access to transportation to attend occasional off-site visits, trainings, or meetings.

Nice To Haves

  • Being bilingual Spanish, Cantonese, Vietnamese and/or Somali is a highly preferred skill for the position.
  • Deep familiarity with King County resources for families and children.
  • Previous experience working within a multidisciplinary medical or pediatric clinic setting.
  • Experience navigating complex social welfare systems and advocating for family rights.
  • Knowledge of Population Health principles and the impact of Social Determinants of Health on clinical outcomes.
  • Interest in advancing health outcomes for all patient populations through the study of healthcare disparities and inclusive care models.
