About The Position

We are looking for a Community Referral Coordinator to join our growing team and help us in our vision to give back lives, reunite families, and heal communities that are suffering from the devastating effects of substance use disorder. Requirements

  • Must reside in the Eugene, OR area.
  • High School Diploma or GED.
  • One year of customer service experience.
  • Ability to sit for long periods of time.
  • Experience with personal computers, word processing and electronic spreadsheets.
  • Remote work from home.
  • Must have laptop or computer and will be provided with a company phone for calls.
  • Periods of stress may occur.
  • Must be in a quiet and private environment.

Nice To Haves

  • Preferably one year or more of experience in office work.
  • Experience with call center,
  • Experience with Community Organizations strongly desired.
  • Associate degree.

Responsibilities

  • Plays a key role by scheduling referred patient appointments, tracking and scheduling circulation rides, and answering questions from community partners (including jails, emergency departments, treatment facilities, others).
  • Receives inbound referrals from community partners in company's CRM and EHR, via phone, chat, or email; processes and schedules, if appropriate.
  • Documents necessary information in patient charts.
  • Gathers new patient demographic and insurance information.
  • Follows up on patient inquiries.
  • Develop trusted relationships with community partners.
  • Understand nuances of community partner referrals and provide accurate responses.
  • Ability to transfer phone calls, provide phone messages and escalate to appropriate staff.
  • Route Ideal Option patient inmate prescription refill requests to prescribing provider.
  • Uses problem solving skills to implement solutions and escalate unresolved problems.
  • Represents Ideal Option professionally and with a positive attitude.
  • Participates in continuous quality improvement activities.
  • Comply with national Protected Health Information laws including HIPAA and 42 CFR Part 2.
  • Works on-call for one week once every 4-5 Weeks.
  • Other projects and duties as assigned.
