About The Position
Grace Community Care & Homes is developing its Central Operations department and seeks a Community Referral and Outreach Coordinator. The role will contribute to person-centered services, reliable operations and accurate documentation.
Requirements
- Healthcare outreach or community-partnership experience
- Strong phone and writing skills
- Organized follow-up
- Respect for patient choice
- Relevant credentials, experience, availability and a resume
- Final regulatory eligibility will be verified before appointment or service delivery
Responsibilities
- Build and verify referral contacts
- Introduce available services to hospitals and community agencies
- Arrange virtual presentations
- Maintain CRM follow-up and track referral conversion
- Work collaboratively with department leadership, intake, credentialing, billing and quality teams
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope
- Participate in orientation, competency review and quality improvement
