About The Position

This is an hourly position that reports to the Community Integration Manager. Main duties and responsibilities include providing community participation opportunities for clients who attend Brian’s House Enterprises day program. This position is considered supervisory and direct care in nature. The Community Participation Coordinator works in conjunction with the CPS Facility Program Manager and Community Integration Manager to meet the needs of BHE clients.

  • Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain, bend, squat, stretch, reach, push up, and stand for several hours at a time.
  • Associate’s degree.
  • At least Two (2) years’ experience in the human service field or equivalent preferred.
  • At least one (1) year of supervisory experience preferred.
  • Proficiency with Microsoft Office; Excel and Outlook.
  • Valid PA. driver's license and reliable transportation.

Responsibilities

  • Procure opportunities which match client's interests and needs.
  • Establish relationships with businesses through telemarketing and direct contact networking to develop creative community integration opportunities for our individuals.
  • Assists managers with development of day trips, activities, and volunteer opportunities for our clients.
  • Ensure that CPS groups have necessary supplies like water, sunscreen, first aid kit, bugs spray, change of clothes, travel wheelchair and any other established CPS outing kit for the health and safety of our clients and staff.
  • Work in conjunction with Maintenance team and Administrative Assistant to ensure that CPS fleet vehicles are in good condition, vehicles are serviced & inspected, track vehicle mileage & review EZpass & gas report provided by accounting and report any issues to Community Integration Specialist Manager.
  • Responsible for CPS cell phones to ensure that they are charged, locked up when not in use.
  • Performs community search outreach and analysis; makes recommendations as required.
  • Provide assistance with barriers such as transportation, including transporting of individuals.
  • Supervises staff & clients in the community and provides guidance to staff & clients as needed.
  • Develops support systems to meet participant needs by identifying and coordinating available services needed to help support community participation.
  • Monitors and tracks community outing destinations, staff/ client group or 1:1 assignments, in community and in facility times for billing purposes.
  • Coordinates with state, local, community and private employment/ recreational resources to assure participant access.
  • Establishes and maintains complete records as required.
  • To report weekly or bi-weekly to Community Integration Manager and CPS Facility Program Manager to discuss, itinerary, scheduling, performance, planning, course of action, and provide written reports to all related client issues.
  • Compiles and maintains supporting documentation including internal and external reports and other identified items such as daily, weekly and monthly checklist for each activity/ outing.
  • Provides direct care to clients and will be considered part of staff: client ratio when needed in the facility or in the community and or virtual.
  • Responsible for supervising staff and reporting both exemplary performance and sub standard performance.
  • Responsible for training new hires on how to complete service notes.
  • Work in conjunction with the managers to issue verbal and written corrective disciplinary actions.
  • Participation in interdisciplinary team meetings and trainings when necessary.
  • Responsible for on-site orientation and community participation training for all new CPS Instructors.
  • Assists in maintaining necessary records (Carelogic/clients books etc.) and program documentation as required by Brian’s House Enterprises policies, federal/state/local laws and regulations, and licensing/accrediting bodies.
  • Participating in and ensuring the implementation of Individual Support Plans and program activities.
  • Perform other duties as assigned.
