Requirements

Knowledge of local community-based agencies i.e. experience conducting health screenings and community presentations in the city of Perris and neighboring communities!
Bilingual (English/Spanish).
Associate degree in healthcare or related field, or 2+ years of equivalent experience in outreach in healthcare customer service.
Availability to work outside of standard hours (e.g., evenings, weekends)
Strong public speaking and presentation skills to engage with community members
Ability to build relationships across diverse populations.
Will require to use own vehicle to travel daily to outreach venues.
A California Driver's License, proof of automobile insurance and a clean driving record is required (no tickets or accidents) is required.

Responsibilities

Engaging with underserved populations and connecting them to essential health services.
Coordinating and attending community events.
Distributing educational materials.
Collaborating with internal departments and external agencies to promote health awareness and increase patient enrollment.
Reducing barriers to care, improving health outcomes, and fostering trust between TrueCare and the communities we serve.
