Community Outreach Worker I
About The Position
Makes visits to homes and other sites promoting services, scheduling appointments and any necessary follow-up outreach services and follow-up services for potential clients and individuals already in the programs such as Well Child, Family Planning, Aging, TB and WIC. This role involves explaining services of the Local Health Department (LHD), assisting clients with transportation and ensuring attendance at appointments. The worker may also conduct initial interviews, provide support, and assist in the provision of services for programs like Aging, Well Child, Family Planning, and TB screening.
Requirements
- High School Diploma or GED.
- Possess a valid motor vehicle operator’s license.
- Submit to a drug screening test and background check.
Responsibilities
- Makes visits to homes and other sites promoting services.
- Schedules appointments for potential clients and individuals already in programs.
- Provides necessary follow-up outreach services.
- Follows up with potential clients and individuals already in programs.
- Explains services of the LHD.
- Assists clients in making transportation arrangements to receive services.
- Ensures client patient attendance at scheduled appointments.
- Follows up by letter, phone call or home visit on missed appointments or referrals.
- May conduct initial interview with patient obtaining factual information.
- Provides support and encouragement to patients.
- Assists in resolution of problems.
- May assist in the provision of services to the patient for services such as Aging, Well Child, Family Planning, TB screening, etc.
