The Community Outreach Navigator is responsible for helping community residents obtain access to healthcare and health impacting resources. The Community Outreach Navigator supports providers and clinical support staff through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. Navigators are highly trained communicators and subject matter experts, skilled in Motivational Interviewing and responsible for ongoing community resource knowledge. They collaborate extensively within the larger community to overcome logistical barriers, increase care quality, improve care continuity, and reduce care costs. Navigators provide outreach opportunities for community residents who lack access to healthcare and health insurance through community engagement and enrollment and application follow-up assistance. This role also builds and strengthens enrollment collaborations with local partner organizations.
