About The Position

This is intermediate-level work performing a variety of tasks under limited supervision where general policies, practices and principles can be applied to situations not previously covered. The job requires a general knowledge of client centered counseling techniques and social work principles and practices. Incumbents may use skill in recommending routine changes in standardized operating procedures. This position will be the primary driver for the Mobile Medical Unit (MMU). This position is responsible for health education counseling as a part of STD and HIV counseling, testing, and linkage to care, treatment, PrEP, or other activities within the Division of Disease Control, STD and HIV Program. Service delivery may occur within the CC Clinic, Mobile Medical Unit, or other clinics or settings in the community. Addresses priority risk factors and prevention, including PrEP, Rapid Access to Treatment (Test and Treat), prevention with Persons living with HIV. This position will participate in outreach screening and community education activities. Responsibilities will be performed in compliance with Disease Control Programs and timeline.

Requirements

  • A current, valid, and unrestricted driver’s license, and have reliable personal transportation required.
  • At least 1 year of experience in HIV/STD field with outreach, testing, and education
  • A current and valid driver’s license to operate the mobile medical unit.
  • Must be available to work Monday through Friday, 8:00am to 5:00pm.
  • Complete 500/501 HIV Counseling and Testing class within 3 months of hire.
  • Completion of STD PASSPORT training class within 6 months of hire.
  • Knowledge of community resources.
  • Knowledge of HIPAA.
  • Knowledge of Sexually Transmitted Infections and HIV.

Responsibilities

  • Provide excellent customer service. Serve all assigned and walk-in clients unconditionally, without partiality and with a positive and pleasant attitude.
  • Provide education on infections that are spread through sexual contact
  • Provide education on all services offered by DOH-Duval
  • Conduct rapid HIV tests and completing pre/post counseling
  • Registering clients into our HMS system and conducting basic eligibility criteria
  • Outreach in the surrounding community to inform of the mobile medical unit (MMU), as well as drive the MMU.
  • Help clients increase health literacy.
  • Work with medical and non-medical case managers, eligibility specialists and clinical staff as part of a care team.
  • Call clients to remind them of upcoming medical appointments and lab appointments and follow-up with the client after these appointments, including program eligibility and re-eligibility.
  • Contact clients as directed to discuss missed appointments, no-shows and identify barriers to care.
  • Document activities in City and State CAREWare, Linkage to Care Module, STARS, HMS, computer, or handwritten forms.
  • Enter client information into computer programs and maintain client files.
  • Work with clients who are new or returning to care (assist Linkage and Surveillance staff).
  • Travel to meet clients within the community.
  • Perform related duties as assigned by immediate supervisor.
