About The Position
Boundless Home Health is committed to providing exceptional, compassionate care to patients in the comfort of their homes. We are seeking a motivated, relationship-driven Community Liaison to expand our referral network and strengthen partnerships throughout the community.
Requirements
- Minimum of 2 years of Home Health marketing experience required.
- Established book of business and existing referral relationships within the Home Health industry is required.
- Excellent communication, networking, and relationship-building skills.
- Self-motivated with the ability to work independently.
- Valid driver's license and reliable transportation.
Nice To Haves
- Knowledge of the local healthcare market is strongly preferred.
Responsibilities
- Build, develop, and maintain strong relationships with referral sources.
- Meet and exceed monthly patient admission goals.
- Coordinate with hospitals, inpatient rehabilitation facilities, skilled nursing facilities, physician offices, and other healthcare providers to facilitate patient care and secure referrals.
- Conduct regular in-person visits to referral partners within the assigned territory.
- Represent Boundless Home Health professionally at networking events and community functions.
- Be available for phone calls and attend in-office meetings as required.
- Maintain accurate records of referral activities and provide regular updates to leadership.
