About The Position

The Community Integration and Transport Mentor will assist with transporting clients to and from Adult Day Programs on a fixed route. When not driving clients on their fixed route, the incumbent will assist staff in effectively implementing Services Plans (SPs) and program techniques through community inclusive activities.

Requirements

  • A High School diploma or equivalent preferred.
  • One (1)year experience driving a 14-passenger bus is preferred but not required.
  • CPR/First Aid training or willingness to obtain training CPR/First Aid training within the first 30 days.
  • Medication Administration Qualification (or complete training within 30 days).
  • Previous experience with mild to severe developmentally disabled adolescents or adults is helpful.
  • The incumbent must be able to respond to special requests and be flexible with regard to the job, tasks, and duties.
  • The incumbent must have good physical stamina/endurance to stand for extended periods of time.
  • The incumbent must also demonstrate empathy and concern for individuals with developmental disabilities and have a passion for working to maximize their independence and personal growth.
  • The Community Integration and Transport Professional must also conduct self in a professional manner and maintain a professional appearance that enhances the agency’s reputation. The incumbent must be able to work effectively as a team member with the rest of Community Program staff.
  • The incumbent will be required and must have the ability to drive for company business.
  • Per auto vehicle insurance carrier requirements: For applicants/employees to qualify for inclusion on the Goodwill driver insurance schedule, for purpose of driving Goodwill fleet vehicles, or for purpose of receiving company mileage reimbursement they must be at least: 21 years of age (not engaged in passenger transportation), 25 – 70 years of age for all passenger transportation services, 25 years of age for CDL.
  • Applicants/employee will be required to undergo a Motor Vehicle Record (MVR) check and background check; applicants/employees must disclose all moving traffic violations or vehicle crashes (within the last five (5) years)(Note: not all violations are a disqualification for employment and each case will be reviewed by the insurance carrier); must have valid Colorado State driver license; must also be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.
  • Applicants/employees using their personal vehicle for company business (receiving company mileage reimbursement), must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition.

Responsibilities

  • Transportation:  The Community Integration and Transport Mentor will pick up and drop off clients at their home and deliver them to and from their Adult Day Program in a safe and timely manner in the company’s vehicles. The incumbent must be able to alter their route according to daily pick-up schedule, adjusting for the safest and most efficient course to deliver clients to their program and back home. The incumbent must be able to read and understand maps and use a GPS (Global Positioning System) and follow all rules and regulations driving in the state of Colorado. The Community Integration and Transport Mentor is responsible for the cleanliness of the vehicle’s interior and exterior. The incumbent will complete daily inspections on the bus before driving it. The incumbent will ensure the bus is in good maintenance at all times and schedule any maintenance for the bus as needed. The incumbent will ensure all clients utilize seatbelts at all times, as well as assist clients on and off the bus at all times. The incumbent will ensure the vehicle’s fuel tank is at least half-full at all times.
  • Community Integration: The Community Integration and Transport Mentor will work with Day Program team members to schedule community activities coinciding with curriculum. All community integration activities will be person centered related to individual goals. The incumbent will collaborate with various community members to ensure client success in the community is fulfilling and safe. The incumbent will also assist the Integration and Employment Operations team in planning and scheduling reverse integration activities into the facility. The Community Integration and Transport Mentor will ensure proper documentation is completed each time community activities are completed. All documentation should be person centered around individual goals.
  • Client Care: The Community Integration and Transport Mentor helps implement program plans, give reinforcement, and provide individual assistance or training. The incumbent will communicate pertinent information to Community Program staff and assist program participants with transitioning throughout the day. The incumbent will also teach proper self-help skills and appropriate interactions through role modeling and programmatic interventions. The Community Integration and Transport Mentor will also assist with medication administration when needed. The incumbent will be an active member of the Day Program team and will assist team members with all client care and activities of daily living. The incumbent will ensure that participants’ supervision levels are maintained in the facility and while out in the community.
  • Client/Program Tracking Reporting: The Community Integration and Transport Mentor will fill out all necessary progress reports, per client, necessary for billing and tracking. The incumbent will assist Case Mangers with completing assessments by providing vital information needed on program participants. The incumbent will conduct and document vehicle inspection sheets of the vehicle prior to operating and report any vehicle deficiencies to the supervisor immediately.
  • Relationship Management:  The Community Integration and Transport  Mentor will exhibit sound judgement when speaking with and assisting participants. The Community Integration and Transport Mentor will communicate in a timely manner and keep supervisor(s) informed of progress, considerations, and pertinent information. The incumbent will deal effectively and calmly with non-routine situations following accepted guidelines and perform other duties as required. The incumbent may need to communicate with parents, providers, or Host Home Providers, as assigned by Case Manager or supervisor.
