About The Position

This is a high-volume practice offering a rewarding, team-oriented work environment. As a Community Healthcare Worker, you will advocate for your client and community, identify and help address health inequities and barriers to good healthcare. You will meet with clients in their homes and at community events, provide social service referrals, general wellness education, and transport clients to health-related appointments. You will also connect patients and families with community resources, follow up with clients and agencies as appropriate to document use/success of referral, and document in the electronic medical records. Assists with follow up and scheduling of appointments as needed. Completes necessary reports and maintains records of data as required. Performs other duties as assigned.

Requirements

  • Valid High School Diploma or GED.
  • Ability to work independently.
  • Maintain confidentiality.
  • Excellent Problem-Solving Skills.
  • Written and verbal communication skills.
  • Knowledge of community resources and experience with high-risk populations.
  • Ability to work collaboratively with the community, patients, families and other members of health care teams.
  • Valid Driver's License and ability to own insured vehicle.
  • Familiarity of basic computer skills, word processing, and data entry.
  • Experience with basic office skills such as filing, scheduling, and customer service.
  • Ability to obtain BLS certification.
  • Willingness to obtain state CHW certification.
