About The Position

This is a full-time position in a high-volume practice offering a rewarding, team-oriented work environment. The Community Healthcare Worker will advocate for clients and the community, identify and help address health inequities and barriers to good healthcare, meet with clients in their homes and at community events, provide social service referrals, provide general wellness education, and transport clients to health-related appointments. The role requires knowledge of community resources, connecting patients and families with these resources, and following up to document referral success. Responsibilities also include documenting in the electronic medical records, assisting with follow-up and scheduling of appointments, completing necessary reports, and performing other assigned duties.

Requirements

  • Valid High School Diploma or GED.
  • Ability to work independently.
  • Maintain confidentiality.
  • Excellent Problem-Solving Skills.
  • Written and verbal communication skills.
  • Knowledge of community resources and experience with high-risk populations.
  • Ability to work collaboratively with the community, patients, families and other members of health care teams.
  • Valid Driver's License and ability to own insured vehicle.
  • Familiarity of basic computer skills, word processing, and data entry.
  • Experience with basic office skills such as filing, scheduling, and customer service.
  • Ability to obtain BLS certification.
  • Willingness to obtain state CHW certification.
