About The Position

This is a high-volume practice offering a rewarding, team-oriented work environment. The Community Healthcare Worker will advocate for clients and the community, identify and help address health inequities and barriers to good healthcare, and connect patients and families with community resources. This role involves meeting with clients in their homes and at community events, providing social service referrals and general wellness education, and transporting clients to health-related appointments.

Requirements

• Valid High School Diploma or GED.
• Ability to work independently.
• Maintain confidentiality.
• Excellent Problem-Solving Skills
• Written and verbal communication skills.
• Knowledge of community resources and experience with high-risk populations.
• Ability to work collaboratively with the community, patients, families and other members of health care teams.
• Valid Driver's License and ability to own insured vehicle.
• Familiarity of basic computer skills, word processing, and data entry.
• Experience with basic office skills such as filing, scheduling, and customer service.
• Ability to obtain BLS certification.
• Willingness to obtain state CHW certification.

Responsibilities

• Advocate for your client and community.
• Identify and help address health inequities and barriers to good healthcare.
• Meet with clients in their homes and at community events.
• Provide social service referrals.
• Provide general wellness education.
• Transport clients to health-related appointments.
• Connect patients and families with community resources.
• Follow up with clients and agencies as appropriate to document use/success of referral.
• Document in the electronic medical records.
• Assists with follow up and scheduling of appointments as needed.
• Completes necessary reports and maintains records of data as required.
• Performs other duties as assigned.
