About The Position

This is a full-time position at the CVHS Crimson Clinic in Petersburg, VA. The role is within a high-volume practice that offers a rewarding, team-oriented work environment. The Community Healthcare Worker will advocate for clients and the community, identify and address health inequities and barriers to healthcare, and connect patients and families with community resources. Responsibilities include meeting with clients, providing social service referrals and wellness education, transporting clients to appointments, and documenting interactions in the electronic medical records.

Requirements

• Valid High School Diploma or GED.
• Valid Driver's License and ability to own insured vehicle.
• Familiarity of basic computer skills, word processing, and data entry.
• Experience with basic office skills such as filing, scheduling, and customer service.
• Ability to obtain BLS certification.
• Willingness to obtain state CHW certification.

Responsibilities

• Advocate for your client and community.
• Identify and help address health inequities and barriers to good healthcare.
• Meet with clients in their homes and at community events.
• Provide social service referrals.
• Provide general wellness education.
• Transport clients to health-related appointments.
• Connect patients and families with community resources.
• Follow up with clients and agencies as appropriate to document use/success of referral.
• Document in the electronic medical records.
• Assists with follow up and scheduling of appointments as needed.
• Completes necessary reports and maintains records of data as required.
• Performs other duties as assigned.
