About The Position

IMPaCT Care is seeking a Community Health Worker to join their team, working directly for IMPaCT Care as an employee embedded within their partnership with Kaiser Permanente in Atlanta, GA. This full-time role involves being a vital link between high-risk patients and the care and services they need, operating in the community rather than behind a desk. The position emphasizes building trust through consistent presence, culturally aligned communication, and hands-on problem-solving. The Community Health Worker will identify and address barriers to care such as food insecurity, transportation, and housing, connect patients to care teams and community resources, and follow up to ensure needs are met. Responsibilities also include documenting interactions, insights, and patient needs using digital tools, utilizing feedback and performance data for growth, documenting client interactions in the EPIC EMR System, completing SDOH screenings, updating care plans, and participating in integrated care huddles with clinical teams.

Requirements

  • High school diploma or equivalent
  • 1–3 years in community-based outreach, health education, or related work
  • Valid driver's license
  • Must own a vehicle
  • Must have car insurance
  • Ability to pass background check and pre-employment screening
  • Flexibility to cover multiple locations across the Atlanta area
  • Applicants must be U.S. citizens.

Nice To Haves

  • Experience with an EMR system such as Epic
  • Familiarity with Medicaid, D-SNP, waiver programs, and recertification cycles
  • Experience with SDOH (Social Determinants of Health ) screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) or case management platforms
  • Knowledge of HEDIS (Healthcare Effectiveness Data and Information Set) metrics or risk stratification concepts
  • Strong ties to community organizations: faith institutions, shelters, food banks, or mutual aid networks

Responsibilities

  • Document all client interactions, needs, assessments, and outcomes in EPIC EMR System
  • Complete SDOH (Social Determinants of Health )screenings and update care plans in a timely manner
  • Participate in integrated care huddles and collaborate with clinical teams
