About The Position

The Community Health Worker (CHW) works with patients in the hospital and in the community to help them get the care and services they need. The CHW serves as a trusted link between patients, the hospital care team, and community organizations. This role focuses on identifying social needs such as food, housing, transportation, and safety concerns. The CHW connects patients to helpful resources, supports care plans, and helps improve overall health outcomes. The CHW works closely with nurses, providers, case managers, and other hospital staff.
