The Community Health Worker (CHW) serves as a trusted liaison between community members, healthcare providers, and social service organizations. The CHW works as an integral, mostly remote member of an interdisciplinary pod Care Team alongside Lead Care Managers, MAs, and RNs. Under the general supervision of Enhanced Care Management Leadership, the CHW conducts outreach, provides health education, addresses social determinants of health, and connects members to medical, behavioral health, and community-based resources. The role works closely with members, families, care teams, and community partners to improve health outcomes, promote self-sufficiency, and reduce barriers to care. It requires strong communication skills, cultural sensitivity, and a passion for serving diverse populations. The position primarily supports direct member engagement alongside Lead Care Managers, while also making in-person community visits and reporting to the office as needed. The role serves diverse populations, including people experiencing homelessness, people at risk of avoidable hospital or emergency department use, people with mental health or substance use needs, individuals transitioning from incarceration, people at risk of long-term care institutionalization, and children, youth, and pregnant or postpartum individuals with specified care needs. The position requires a high school diploma or GED, at least one year of experience in case management, member care, customer service, call center, or member care, CHW certification, and a valid California driver's license and vehicle insurance. Employees must be able to travel in a personal vehicle for outreach visits and have reliable high-speed internet and a private, HIPAA-compliant home workspace. Pay is $25–$28 USD. Full-time employees are eligible to enroll in benefits after 60 days of employment; 401(k) eligibility is effective after one year.
