About The Position

The Community Health Worker (CHW) serves as a trusted liaison between community members, healthcare providers, and social service organizations. As an integral, mostly remote member of an interdisciplinary pod Care Team, the CHW works alongside Lead Care Managers, medical assistants, and registered nurses to conduct outreach, provide health education, address social determinants of health, and connect members with medical, behavioral health, and community-based resources. Under the general supervision of Enhanced Care Management Leadership, the CHW works with members, families, care teams, and community partners to improve health outcomes, promote self-sufficiency, and reduce barriers to care. The position involves direct member engagement, including in-person community visits, and requires strong communication skills, cultural sensitivity, and a commitment to serving diverse populations. The CHW supports people across a range of populations of focus, including individuals experiencing homelessness; people at risk for avoidable hospital or emergency department use; individuals with serious mental health or substance use needs; people transitioning from incarceration; adults at risk of long-term care institutionalization or transitioning from nursing facilities; children and youth enrolled in California Children’s Services; children and youth involved in child welfare; individuals with intellectual or developmental disabilities; and pregnant or postpartum individuals, including the birth equity population. This is a mostly remote role requiring a private, HIPAA-compliant home workspace and reliable high-speed internet. The CHW must travel in a personal vehicle for outreach visits and attend in-person office visits as needed. Mileage is reimbursed at a standard rate. Full-time employees become eligible to enroll in benefits after 60 days of employment; the 401(k) benefit is effective after one year.
